I e-mailed the CIW people and this is what I got back from them. I had filled out the Credit Application Form and didn't know where to send it to. Also, the link is another form you have to fill out.

The fax number is 512-328-5237 and once your information is received and you
sign the CIW Certification Agreement you should receive your certificate
within 6 weeks. Here is link with this information: http://www.ciwcertified.com/exams/examcredit.asp

I download the doc like the link said but didn't know about the "other" form until they e-mailed me saying I needed to fax a copy of an "agreement"....

BUT!!!! You can have the credit....

Well I just sent mine a week ago. But after awhile i'll e-mail them every single day and then i'll call their 1-800 number until they feel the urge to send me something....
Somehow I think they will get the point....

I emailed the CIW Certification team
about myrequest and this is what I got
back from them;
