Exam Structure

I recently wrote my Net+ and found it to be a mix of knowledge and troubling shooting questions.

I have a copy of TestKing's practice exams and it seems to just cover real life situations... Is that how the exam is structured also?

Microsoft is pretty vague on there site with regards to what to expect.

How long is the exam? How many questions? I take it a pass is 700 but what is the high end cap set at?
Thanks.
