In the following string :

Provider=SQLOLEDB.1;
Persist Security Info=False;
Data Source=[serverName];
Initial Catalog=[databaseName];
User ID=[username];
Password=[password];

Can someone please tell me where exactly I get the Username and Password
from?

1.Ask you Database Administrator.
2. Go to Security folder in the Enterprise Manager and create new login and
user. Than assign this username and password to your connection string.
