Bad passwords. Being logged on to multiple machines and changing the
password and forgetting to log off those other machines.
Depending on what SP you run with W2K, there's a problem where the
invalid password count is not replicated after it's reset on a DC.
That'll cause a lockout if one of those un-updated DC's is used as the
authentication server and the user enders a bad password (even once!).
