I find this topic quite hard to get into. I understand CA and digital certificates but it's hard practising with this stuff.

Is there many questions on this or is it one of those fringe topics that you will get 2-3 questions on which won't be too difficult.

Not an easy question 'cause Micro$oft has many exam pools. Maybe when you take your test you could recieve a test that includes just 1 or 2 CA related questions....maybe you won't recieve any CA...maybe you could recieve more...

Anyway, i've seen in this and other forums that people say they do not recieve too much questions about this topic, at least, less than DNS or RAS questions. So, if you understand well the theory...you should be ok for the real test.

For 216, understand the CA hierarchy, when to use Enterprise (For internal AD), when to use Stand Alone(with Third Party CA for code signing to the public, etc.). Understand, what happens at expiration and about the CRL. I had several questions in Certificate Services for 216.

At one point it was a fringe area, however with the number of question pools and the more recent conciousness of security, it might have more than a few questions.

Like JB stated, the concepts are pretty simple and cut and dry. It is the successful implementation that is a bear.
